The deadline for the January, 2006 Training Class is November 28, 2005. You may mail, fax or email this application. Please type single space in black ink only, and/or submit application on plain, lined, white paper in black or blue ink. After your application has been received, you will be asked to schedule a personal interview/workout to discuss your enrollment in the program.

Please note that entrance into the training program requires Intermediate level Pilates experience on all equipment; if you are uncertain about your eligibility, please contact Jennifer Campbell prior to applying.
